Now, a fair caveat. The quality of the underlying studies was rated low across most of the review, and confidence in the results varied depending on the outcome. This isn't a bulletproof case yet. The findings are most reliable for people who already have a diagnosis and are trying to understand their risk trajectory. For otherwise healthy adults, the picture is fuzzier — though the fuzziness runs in the direction of "probably still useful," not "meaningless."

Why This Matters If You're Not Sick
Here's the thing about most standard heart health markers: they tend to become interesting right around the time they've already become a problem. Cholesterol climbs. Blood pressure creeps. You get the news at 52 that your arteries are not what they used to be. Meanwhile, CRF has probably been drifting downward for a decade, and nobody was watching.

These aren't lab-grade, but they're plenty accurate for watching trends — which is the point.
If you want something even simpler, the Cooper 12-minute run test has been used for decades. You run as far as you can in twelve minutes, on a flat surface, and the distance gives you a rough VO₂ max score. It's brutal and honest. A one-mile walk test works too, if running isn't on the menu.
