Most of us have a workout we default to. Maybe it's a Sunday long run, a lunchtime lift, or the sweaty 20 minutes on the bike where you swear you'll never do intervals again. Whatever your thing is, you probably assume it's doing your heart and blood vessels some good. It is. But here's the wrinkle: when researchers actually stack different types of exercise against each other and measure what's happening inside your arteries, no single workout comes out on top for everything.
A recent pooled analysis of 51 randomized trials looked at exactly this question — which style of training does the most for vascular health? The answer turned out to be refreshingly unglamorous. Different workouts help in different ways. Which means your instinct to mix things up may already be smarter than any single-method program you've seen advertised.
What the researchers actually did
The analysis pulled together 51 trials involving 2,638 adults between 18 and 65, all classified as overweight or obese. Each intervention ran at least four weeks, and researchers measured blood vessel health before and after — not just whether people felt better, but how their arteries were actually behaving.
They grouped the exercise programs into five categories: steady-state cardio (think jogging, cycling, brisk walking), strength training, interval training, combined cardio-and-strength routines, and hybrid programs that mixed several modalities together. Then they compared the workout types against each other, not just against sitting on the couch. That's the useful part. Most exercise studies only tell you that moving is better than not moving. This one asked a sharper question: better at what, exactly?
Three ways to measure a healthy artery
Vascular health isn't a single number. The researchers zeroed in on three distinct measures, each capturing something different about how your blood vessels are doing.
- Flow-mediated dilation (FMD) — how well your vessels widen when blood flow picks up. Think of it as arterial responsiveness.
- Pulse wave velocity (PWV) — a measure of arterial stiffness. Softer, more elastic arteries expand and recoil with each heartbeat. Stiffer ones don't, and that's associated with worse cardiovascular outcomes over time.
- Carotid intima-media thickness (CIMT) — the thickness of the artery walls in your neck. This tends to reflect longer-term structural changes and is a decent proxy for the slow drift toward atherosclerosis.
A workout could theoretically nudge one of these numbers without doing much for the others. And that's more or less what the researchers found.

Which workout won which category
Here's the fun part. Each style of training had a specialty.
Hybrid training — the everything-and-the-kitchen-sink approach — produced the biggest improvements in blood vessel responsiveness (FMD). If your program cycles through cardio, resistance work, and something high-intensity in the same week, your vessels seem to get better at reacting to demand.
Interval training came out ahead for reducing arterial stiffness. Repeatedly asking your cardiovascular system to shift gears — hard effort, recovery, hard again — appears to keep your arteries more elastic. This one tracks with what a lot of cardiologists have been saying for years about the benefits of short, hard bursts.
Steady cardio and combined training (cardio plus strength) had the biggest impact on artery-wall thickness. These are the styles most likely to reflect long-haul structural improvement — the slow, boring kind of change that matters most in your 50s and 60s.
Strength training on its own didn't top any of the three categories, but that's not the same as saying it's useless for your heart. It's just that the specific vascular measures used here favor modalities with a heavier aerobic component. Lifting still earns its place for a dozen other reasons — muscle mass, insulin sensitivity, bone density, the ability to carry your own groceries at 75.
Why one workout can't do it all
Different training styles put genuinely different stresses on your cardiovascular system. Intervals yank your heart rate up and down, forcing vessels to adapt to rapid changes. Longer steady sessions keep blood moving at an elevated pace for extended stretches, which seems to encourage structural remodeling. Strength work spikes blood pressure briefly and creates its own kind of vascular stimulus. Each of these is a different signal to your body, and your body responds accordingly.
That's probably why no single workout dominates. Blood vessels aren't a single tissue with a single job — they need to be responsive, elastic, and structurally sound, and those qualities respond to different inputs. Chasing one metric with one workout is a bit like eating only broccoli because it's a superfood. Fine, but incomplete.
Worth noting: the researchers themselves flagged that confidence in the evidence was low, and some comparisons rested on limited or inconsistent data. This isn't a definitive ranking. It's a signal, not a verdict. Still, the direction it points feels reasonable — variety helps because variety is what your cardiovascular system is built to handle.

What this might look like in a real week
You don't need to blow up your routine because of one meta-analysis. If you love your morning runs, keep running. If lifting is what gets you to the gym at all, don't quit lifting. But if you've been doing the same thing for years and wondering whether to shake things up, this is a decent nudge.
A rough weekly shape that touches all the bases might look something like:
- One or two sessions of steady cardio — 30 to 45 minutes at a conversational-but-working pace.
- One interval session — could be sprints, bike intervals, or a hard hill workout. Short and unpleasant beats long and comfortable here.
- Two strength sessions — full body, or an upper/lower split. Heavy enough to matter.
- Maybe one hybrid or circuit-style day where you blend modalities.
That's five days of movement, with plenty of room for a rest day or an easy walk. Not a magic formula. Just a spread of stimuli that gives different parts of your cardiovascular system something to work on.

The bigger picture on movement and long-term health
There's a growing body of research connecting fitness earlier in life to healthier arteries decades later. Your cardiovascular system has a long memory. What you do in your 30s and 40s shapes what your arteries look like in your 60s and 70s. That's not meant to guilt anyone — it's just a reminder that the compounding is real.
The interesting thing about vascular health is how much of it happens quietly, in the background, while you're doing something else. You feel the interval workout as burning lungs. You feel the run as tired legs. You don't feel your arteries getting more elastic. But they are, if you keep showing up.
